Health Forms

Muhlenberg College requires all new incoming students (first-year and transfer) to complete essential health forms and records before arriving.

Follow the steps below to ensure you’ve submitted the necessary health information.

Requesting, changing, or canceling an appointment in Health or Counseling:

First, log onto MyLVHN to create your account. Doing so will reduce your wait time when you arrive for your appointment. You will only have to do this once.

• Once you have an account on MyLVHN, then you can request, change, or cancel an appointment via the following:
  • Call 484-664-3199 (Health Center) or 484-664-3178 (Counseling Services).
  • Walk in and request an appointment.

You can no longer request an appointment through the PyraMed health portal or through direct email to a provider. Please reach out again via one of the above options if you have previously sent a request through PyraMed.

Our hours are Monday through Friday, 8:30 a.m.-4:30 p.m., while classes are in session during fall and spring semesters.

After-hours support:

For after-hours counseling concerns, please call 484-664-3178 to speak with a counselor. For health emergencies on campus, please call Campus Safety (484-664-3112) who can also request a Muhlenberg College EMS response.

PyraMed Health Portal

• As your health records will now be maintained in Epic (the electronic health record system used by LVPG and many other health care entities nationally), please DO NOT utilize the PyraMed Health Portal to communicate, upload records, or request appointments for Health and Counseling. This system is no longer in use by Health and Counseling offices.
• Varsity Athletes: Please note that the Muhlenberg College Sports Medicine department will continue to use PyraMed for athletic training only.
